Medihelp Consulting is a small business offering General Practice Pharmacist Services, Home Medication Reviews, Residential Medication Management Review and Quality Use of Medicines Services. I am available to help with medication related advice, assistance and support with all medication related issues. I also provide a Rural Pharmacist Locum Service for rural pharmacies in the Mid North who may require a relief pharmacist. I am happy to travel further if required.       <image:caption>A General Practice Pharmacist works directly and collaboratively with GPs and other health professionals to support the quality use of medicines. From undertaking medication reviews and providing medication advice to patients, to conducting clinical audits and providing education to GPs and other practice staff, a general practice pharmacists supports the quality use of medicines through a variety of activities. Practice consultations, providing advice on medications, and medication aids Education activities with patients, GPs, nurses, administrative staff within practice and allied health Medication management within practice Drug information queries Vaccinations Liaison services between all medication service providers, ie. Aged care facilities, pharmacies and general practices. I work closely in collaboration with Jamestown, Peterborough, Booleroo and Orroroo pharmacies, or your local pharmacy. I am available at Goyders Line Medical Jamestown, Orroroo &amp; Peterborough Monday and Wednesday each week.</image:caption>

      <image:title>SERVICES - Residential Medication Management Review (RMMR) &amp; Quality use of Medicines (QUM)</image:title>
      <image:caption>A Residential Medication Management Review (RMMR) is a service that can be provided to patients who reside in an Australian Government funded Residential Aged Care facility. The aim is to identify, resolve and prevent any medication related problems they may have. The review is conducted when requested by the patients referring Medical Practitioner. This review is then conducted in the Aged Care Facility where the patient resides, in collaboration with the patient, the patient’s family (if requested by the patient), and their healthcare team. ‘The patient is the centre of the review and their preferences are the main focus’. Then the pharmacist generates a comprehensive report that is provided to the referring Medical Practitioner who reviews the report and implements changes as needed. Aspects that are covered in the interview and review process involve; Providing information to the patient about their medications, such as why they take them and how to take them Discussing any concerns they may have with their medications Liaising with the patient’s healthcare providers Reviewing when and how their medications are taken Ensuring that their medications are correctly indicated, and at appropriate dosages. Checking for drug interactions De-prescribing or decreasing the number of medications they are taking. Addressing ‘poly-pharmacy’- an Australian Government health priority area Suggesting alternate or more appropriate medication if necessary and making additional medication suggestions if needed Reviewing their use of complementary medications Discussing issues that arise when taking their medications, such as swallowing difficulties. Two additional follow-up services can be provided to the patient if needed. (Please note there is no charge to patients for this service). Quality Use of Medicines (QUM) is a separate service provided to Australian Funded Aged Care facilities to improve quality use of medicines by improving practice related procedures and policies. This service involves performing education related activities, advice and review of policies and procedures, in conjunction with staff, such as nurses &amp; carers who work at the Aged Care Facility.</image:caption>

      <image:title>SERVICES - Home Medication Review (HMR)</image:title>
      <image:caption>Many people struggle with their medications, they may not know what they are for, how to take them correctly, how to store them, or they may be worried about side effects or they may simply be confused. I am able to help you through a Home Medication review (HMR) through which I can assist you to better manage and understand your medications. This service involves visiting you at home to discuss your medications and medication related management. We look at everything you are taking including prescription medicines, over the counter medicines and vitamins and supplements. We check that everything is being taken correctly, show you how to store your medications properly, suggest ways to help you remember when to take your medications, prescription management and drug interactions. I can show you how to use and look after any medical devices that you may need, such as inhalers, blood pressure monitors and blood glucose monitors. We also discuss your diet, physical activity and lifestyle to better manage your health. It also involves a comprehensive clinical review of your medicines and follow-up report is sent to your GP, and we work together to discuss the best medication plan for you. A HMR may be appropriate for you if you; Are taking more then 5 medications per day Your medications aren’t working the way you think they should be working Are confused or don’t always remember to take your medications If you need to be closely monitored because of the medications you take You have recently started new medication, your medication has changed or you begin to feel unwell after taking medications You are worried about drug interactions You take additional over the counter medications You have recently been in hospital Two subsequent home visits are available following the initial home visit if needed.
